چکیده

Peripheral arterial disease commonly presents as intermittent claudication, which is pain in the calf that occurs on walking and which is relieved by rest. More severe forms of peripheral arterial disease present as rest pain, gangrene or ulceration, occasionally leading to amputation. Abdominal aortic aneurysms commonly remain asymptomatic but sometimes rupture with a high risk of death. Symptomatic aneurysms and large asymptomatic aneurysms require surgical repair because of an increased risk of rupture. Health service issues that are currently important in meeting the needs of patients with these peripheral vascular diseases are, in primary care, the appropriate management of cardiovascular risk factors and referral of patients with intermittent claudication. In secondary care, key issues are the centralisation of services around major vascular units at regional and supra-district level and the minimum facilities that should be provided in these units. In the clinical management of patients, the diagnosis and treatment of claudication, the use of interventional radiology and reconstruction surgery, and the management of asymptomatic aneurysms are important. Also, the value of screening, particularly for asymptomatic aneurysms, has been debated in recent years. A key problem for purchasers at present is the specification of theminimum requirements for providing a vascular service at secondary care level. Whether or not to screen for aortic aneurysms is another important issue.

Diagnosis, Prognosis and Treatment of Peripheral Vascular Disease in Patients with Diabetic Foot Ulcer: Adopted IWGDF Guidance

Introduction: Peripheral arterial disease (PAD) is common in diabetic patients, and about half of the patients with diabetic foot ulcers (DFU) have concomitant peripheral arterial disease (PAD). The purpose of this study was to select an appropriate and comprehensive clinical guide in the management of DFU. The part of the practical guide that pertains to this is based on three systematic revie...

Effect of Heparin Administration during Coronary Angiography on Vascular or Peripheral Complications: A Single-Blind Randomized Controlled Clinical Trial

Background: Coronary angiography consists of the selective injection of contrast agents in coronary arteries. Optimal strategy for heparin administration during coronary angiography has yet to be determined. We assessed the effect of heparin administration during coronary angiography on vascular, hemorrhagic, and ischemic complications. Methods: Five hundred candiates for diagnostic coronary ang...

The Relationship between Abnormal Ankle-brachial Index and Micro-vascular Complications of Diabetes Type II

Background:The prevalence of diabetes mellitus is increasing worldwide and the relationship between peripheral vascular involvement and the complications of diabetes disease, such as retinopathy, has been reported in some studies but has not yet been fully proven. The aim of this study was to investigate the relationship between diabetes complications and peripheral and central vascular involve...

Surgical infrainguinal revascularization for peripheral arterial disease: factors affecting patency rate

Background: Peripheral arterial disease is a source of morbidity and mortality. Surgical vascular reconstruction is a treatment option but probability of failure and complications are important concerns. In this study, we evaluated outcome of surgical infrainguinal reconstruction and factors affecting graft patency for a period of one year. Methods: In this cohort study, 85 consecutive patie...

Initial experience with the benefits of intraoperative angiography in surgery for peripheral arterial occlusive disease.

BACKGROUND This preliminary investigation evaluated the procedures for intraoperative angiography-assisted peripheral vascular revascularization in a teaching hospital. METHODS Between August 2000 and July 2001, intraoperative angiography with or without intervention was applied during surgery for peripheral arterial occlusive disease (PAOD) in 11 patients. Intraoperative angiography was empl...
